1. Exceptional Leave Request
If your child requires a leave of absence for exceptional circumstances, you can download the Exceptional Leave Request form below. This form must be completed and submitted to the school office for approval in advance.

Should your child need to take medication during school hours, please download the Medical Form. Ensure it is filled out with the required information and submitted to the school office with the medication.

3. Registering Your Child for CoolMilk
